2013 Highland Colts Football & Cheer Registration extended to May, 15, 2013.
All registration days held at our Home Field – Manana Community Park –
1310 Waimano Hm Road.
May 15 – 4:30 am to 6:30 pm
Mitey MIte, Jr. Pee Wee, Pee Wee, Jr. Midget & Midget Football
CHEER (ages 5 – 11)
Requirements to register:
1. Parent with a valid ID or legal guardian with proper guardianship papers
(original & 2 copies) must be present to register.
2. Participant must be present for weigh in and signature.
3. Original Birth Certificate & 2 copies, submitted on day of
registration. Originals returned after Oahu Pop Warner Certification
in July.
4. 2 copies of medical insurance card (military 2 copies of ID),
submitted on day of registration.
5. 2012 – 2013 year end report card must be submitted upon
availability with all pages completed. (Report cards are mandatory to
remain registered and to participate. Present original with 2
copies), can be submitted at any future posted / confirmed
registration day, latest on July 17, 2013 / 4:30 – 6:30pm.
6. Physical completed on the official 2013 Pop Warner Form (provided
at registration at Mañana Park once the form is confirmed & provided
by Oahu Pop Warner) physical form must be dated 2013 and signed by
physician.
Due on or before July 17, 2013 / 4:30 – 6:30pm.
7. Cost $350 (minimum deposit $200 required at registration) cashiers
check or cash only.
8. Second payment of $150 due on June 19, 2013, Third payment for
option of $30, if applies, due on July 17, 2013, along with report
card and physical.
